I understand that completing this application and payment of application fee does not guaranteed my child a place at the school.
I certify that the information provided in this form is complete and accurate in so far as I know and that if I fail to provide accurate details
of important information about this child, CVIS has the right to deny admission or withdraw the child’s place in this school.
Should my child be accepted, I agree:
 To authorize the school to conduct necessary emergency medical attention for my child in cases of any injury at school or
during school activities such as field trips and camps.
 To make all due payments in accordance with CVIS schedule of fees.
 That the child and the parents/guardian must comply with the school policies / rules and regulations.
 That my child’s photographs may be taken and used in publications for school promotions such as newsletters, brochures,
websites and social media platforms.
